YOUR VISIT

We want your visit to White Memorial Multi-Specialty Clinic to go smoothly. The information here can help ensure the time you spend at the clinic is both effective and efficient.

  • Well-child exams / Physicals are on an appointment basis.
  • Same day appointments are available for sick children, but we encourage you to make an appointment to better serve you.  

WHAT TO BRING WITH YOU

Photo identification – Adult patients and adults who accompany patients under age 18 must have photo identification, such as a driver’s license, passport, military ID, employment badge, or student identification card.

Immunization card – If appointment is for a well-child exam / physical.

Proof of insurance – Bring patient insurance ard and subscriber insurance card.

+ ADDITIONAL INFORMATION (RECOMMENDED)

  • Medical history information – Bring information about the patient’s past surgeries and medical conditions.
  • A list of the patient’s allergies.
  • A list of the patient’s current prescriptions and over-the-counter medications.
  • A list of questions – If you like, bring a list of questions and a way to record your answers and other information to review later.
  • Any forms you need your clinician to fill out, such as school physical forms.

REGISTRATION

Your first visit with us is usually the longest and we leave the most time in our schedule for it. Please plan to arrive earlier for your first visit to complete necessary registration forms.

In order to save time you can fill online the Health History Form of your child that you will find below, print and bring it with to your visit.

PAYMENT

Any insurance copay is due at the time of your visit. We make paying for your visit easy for you by accepting cash, personal checks, and major credit cards.
