YOUR VISIT

We want your visit to an Adventist Health Endocrinology Center to go smoothly. The information here can help ensure the time you spend with us is both effective and efficient.

Please call us at 805 500-2196 to schedule an appointment.  Your first visit with us is usually the longest and we leave the most time in our schedule for it. Please plan to arrive at least half an hour earlier for your first visit to complete necessary registration forms.

You can fill online the registration form you will find below, print and bring it to your visit.

It is important that you supply us with your:

  • Prior medical records
  • Medications and dosages
  • Physician referral if applicable

What to bring with you

Photo identification – Adult patients must have photo identification, such as a driver’s license, passport, military ID, employment badge, or student identification card.

Proof of insurance – If you plan to pay for a non-work-related injury or illness with insurance, please bring proof of insurance.

Payment

Any insurance copay is due at the time of your visit. We make paying for your visit easy for you by accepting cash, personal checks, and major credit cards.
